Drug Hx:

Tab levothyroxine 0.5mg /Daily

Tab Empagliflozin/metformin 5mg/500mg(TDS) began from the1403

Tab OCP(LD)

Tab Cabergolin1mg(Twice a week)

Amp octreotide 20mg(Twice a month every 14days) began from the 1402

•A 68 year-old-man known case of None Functional Pituitary Adenoma underwent operation 6 years ago (1397.8.7)

PM Hx: MI, ICD(1396)

Drug Hx: Tab Hydrocortisone 15 mg/ Daily

PMH: DM since a 1.5 years ago
DH: Metformin 500 / Glorenta 10/5 / insulin (Toujeo & novorapid)/
Atorvastatin 20
FH: HTN in her mother

ID: A 35-years-old man, married (1children), from zanjan
CC: Headache, Decreased libido, impotency, weight loss of about 20 kg in the last six months
PMH: Negative
DH: Negative
FH: IHD in her father

•A 42-year-old man with decreased libido and impotency since 6 months before diagnosis

q Loss of right eye vision since 1 months before diagnosis

qHe visited an ophthalmologist and due to lesion in brain MRI, he referred to a neurosurgeon in 1398.8

qPMH : negative

qDH : negative

qSH: he addicted to opium since 1 years before diagnosis

case 2

•A 38-year-old pregnant (16 wk) woman with macroprolactinoma

qPMH : secondary amenorrhea since 9 years ago and Galactorrhea following pregnancy since 13 years ago but she had no work-up until she developed loss of right eye vision

qShe has a 13 years old girl

qDH : OCP/cabergoline

qFH : negative

case 1

•ID : A 31-year-old woman, single

• CC : irregular mensturation and galactorrhea since 4 years ago

• No weight gain or headache or hirsutism

•DH : aldactone, cabergoline ( started with 0.5mg/wk and increased up to 10mg /w), metformin

•At the Follow-up: the patient developed amenorrhea, Duphastun was initiated for her and cabergoline dose decreased to 3 mg/wk mensturation happened after 10 days

•FH: NEG

case 2

•PI : a 51- year-old man with history of TSS , 5 years ago ( Non functional macro adenoma 36*25 mm ) referred for follow-up visit

•BMI : 50

•DH : neg

•PMH: negative

•FH: negative

•A 36-years-old woman, married (2 children), from Isfahan

•CC: oligomenorrhea since 5 years ago, Galactorrhea since 15 months ago and headache and blurred vision since 5 months ago.

•No history of Polyuria and nocturia

•DH: OCP (In some months)

•FH: Negative

•Second delivery of the patient: 1397.9.29

•End of breastfeeding: 1399.3.20

•Prolactin: 46 ng/ml (1399.6.30)

case 1

•A 47-year-old woman with unilateral galactorrhea since 4 years ago

•PMH: hypothyroid since 12 years ago

•DH: tablet levothyroxine 50 micg /daily

•FH: hypothyroidism and cerebellum tumor in her mother

case 2

•A 22 years old woman with history of pituitary adenoma resection (Cushing disease) 7 years ago

•She become pregnant in Farvardin 1401 and she had no clinical or laboratory evidence of tumor recurrence before pregnancy

•She gave birth in aban 1401 and then she had a significant increase in her weight so she visited a physician

case 3

•A 44-year-old man referred to an ophthalmologist due to headache and diplopia and MRI was requested for him

•Other symptoms: dizziness, decreased libido since 1.5 years MRI has been

•PMH : negative

•FH : hypothyroidism in his mother and sister

•DH: negative

case 1

ID: 26 years old woman , single, known case of prolactinma

Symptom: irregular mense ,galactorea, headache, Weight Gain, hairsutism, dizziness, ortostatic hypotention

Sign: purple striae ( neg), moon face (neg) , snoring (neg), acromegaloid face (neg)

FH: DM , hypothyroidism

PMH : Hypothyroidsm

DH: cabergoline, levothyroxin

case 2

A 42-year-old woman with diabetes and hypertension since 3 years ago

According to the symptoms and sign and tests, the patient was diagnosed with Cushing's two years ago

Sign and symptom: obesity , menometrorrhagia, moon face , purple striae , abdominal obesity

PMH: HTN , DM , abortion

FH: Neg

DH: valsomix 80/5, metformin 500, liraglutide, atorvastatin 20 , ketoconazole, ferrinject
